It has been my absolute pleasure and honor to be a part of your health and wellness journey. I am deeply grateful for the trust you’ve placed in me and our team over the years. I understand that changes in your health care team can be stressful or inconvenient, and I want to assure you that we are doing everything we can to support a smooth and thoughtful transition.

To obtain or transfer your medical records, fill out this HIPPA Medical Release form. All medical records releases will be fulfilled by Morgan Records Management, LLC.
